Payment

Baby Blossoms Physical Therapy operates on a private-pay, out-of-network basis only. I gladly accept credit cards, HSA/FSA cards. A detailed superbill of services can be provided upon request so you can submit it to your insurance for possible reimbursement. Please note that I do not bill insurance directly. I am happy to answer any questions about this process when you schedule. 

This model allows me to deliver the highest quality, fully customized one-on-one care without the restrictions, paperwork, and time limitations often imposed by insurance companies. By removing insurance involvement, I can focus entirely on your baby’s individual needs, provide truly personalized treatment plans, and offer the flexible, unhurried sessions that infants and families deserve.

My priority is exceptional, therapist-controlled care that puts your child’s progress first.

Wellness sessions are educational and not billed as physical therapy treatment, therefore, superbills are not provided for wellness sessions. 

Pricing information: 

Initial Evaluation- $200

Follow-ups visits- $150

Wellness sessions- $125 for 60 min

*Free 15 min phone consultations to discuss your concerns and see if an evaluation is warranted. 

Location

I provide mobile in-home visits serving Murfreesboro, Smyrna, and nearby areas. 

Travel Fees: 

To keep care accessible while covering the time and costs of home visits, I use the following simple guidelines: 

• No travel fees within a 15-mile radius of the home base. (Will discuss travel fee for those that are in the Smyrna area as travel fees may vary on location and time of day)

• Beyond 10 miles: $20 for every additional 10 miles (or portion thereof).

Cancellation Policy: 

You may be subject to a $50 last-minute cancellation fee/no-show fee for appointments cancelled less than 24 hours' notice to appointment start time. 

I completely understand that babies get sick (and life with little ones is unpredictable). Please reach out as soon as you know you need to reschedule- I am happy to discuss individual situations and waive or adjust fees as appropriate.
